CHILD SCREENING - 5 YEARS

Speech Language Skills

 Follows directions with spatial concepts, "in front, in back, next to, under."
Understands "long" vs. "short," such as, "show me the girl with long hair."
Identifies 2 different shapes, "square, circle, triangle"
Repeats 5-8 word sentences, such as, "They are washing the dog."
Uses "he" and "she" correctly
Counts up to 10
Uses sentences with at least 5 words
Uses past tense, verb + -ed or irregular form, "washed, caught"
Names categories, "food, toys,"
                      *Refer to a Speech Language Pathologist if the child has less than 8 items checked

        Check if the child can produce each of the following sounds correctly
